New "doctor doctor" joke finally grasped by cardinal (6)

Def = New

Doctor = MO
Doctor, joke finally grasped = DR with E grasped (i.e. DER)

Clever treatment I think but rubbish

In Malton the German says "Out with the old and in with the new" (6)

MALTON minus ALT = "old" in German with DER ("the" in German) inserted. New = def

On = RE
Occasion=Do
Minute = M
Up = reversal indicator

Article in German appearing in The Scotsman and in Vogue (6)

DER inside MON, def = in vogue, The Scotsman is a newspaper used to wrap the chips that are served with deep fried Mars bars and Vogue is something women "read" at the hairdresser's.
